About Your Visit

What you should bring to your Initial Consultation at Foothills Pain Management Clinic:

  • Please arrive 20 to 30 minutes prior to your scheduled time to complete all required patient registration forms. To save time, you may download them by clicking here, and bringing the completed forms with you. You may also request that we fax them to you.
  • Insurance Card: Bring your insurance card of your ACTIVE insurance. If you have an HMO and have an authorization please bring the authorization as well.We will verify your benefits for the visit; however it is your responsibility to know your benefits including proper Co-pay or Deductible, as it is REQUIRED in full before you can be seen.
  • Forms of payment are: Cash; Check; Visa; MasterCard and American Express.
  • Picture ID: Bring a valid form of identification showing a current picture.
  • Medication: We will need a CURRENT list of all Medication you are currently taking or have tried in the past. Please bring the bottles (especially pain medication) with you to your appointment.
  • A list of questions you may want discussed with the doctor regarding your condition and/or treatment.
  • Previous medical records pertaining to what you are being seen for. This will include imaging reports (X-Ray, MRI, and CT).

Follow-Up Visits and In-Office Procedures:

  • We will verify your benefits for the visit; however it is your responsibility to know your benefits including proper Co-pay or Deductible. As it is REQUIRED in full before you can be seen.
  • Medication:  We will need a CURRENT list of all Medication you are currently taking or any changes you may have had to your medication list.  Please bring the bottles (especially pain medication) with you to your appointment.
  • A list of questions you may want discussed with the doctor regarding your condition and/or treatment.

For In-Office Procedures Please abide by the following Pre-Procedure Instructions:

  • PLEASE NOTIFY THE DR OR OFFICE IF YOU ARE TAKING ANY BLOOD THINNING MEDICATIONS. You MUST STOP all Blood Thinning Medication 7 Days Prior to Procedure.
  • It is your responsibility to arrange in advance for a responsible adult to drive you home following your procedure.  You cannot drive yourself if you have had general or spinal anesthesia or sedation.
  • Notify Dr. Mehta before coming to the office if there is any change in your physical condition, such as a cold, fever, persistent cough, or the possibility of pregnancy.
  • If you wear glasses, please bring a case for their safekeeping.
  • DO NOT DRINK OR EAT anything after midnight – not even coffee or water.  If you normally take any medication, please take it with a small sips of water unless otherwise instructed.  You may brush your teeth, remembering not to swallow anything.  Refrain from the use of mints, chewing gum or cigarettes.  This will reduce the possibility of nausea and vomiting following anesthesia.
  • Please bathe or shower prior to your procedure.  Remove all makeup.  Do not use lotions or oils after bathing.
  • Wear loose, comfortable clothing such as sweat suits and easy-to-button shirts or blouses. Wear comfortable shoes ; no high heels, please.
  • Leave all valuables, including jewelry and cash, at home.  We cannot be responsible for damaged or lost property.
  • Bring ALL medication in original bottles to the office on the day of your procedure.
  • Be prepared to sign a form giving your consent for the procedure.
  • Your family will be asked to wait in the lobby until you are in recovery.  You will be in the office for approximately 20 – 40 minutes following your procedure.
  • Before you leave the office you will be given written instructions for your care at home.
  • Do not drive a car, smoke, drink alcoholic beverages, operate machinery or cook for 24 hours after your procedure.

Post-Op Procedures are as follows:

  • Before you leave the office you will be given written instructions for your care at home.
  • Do not drive a motor vehicle, operate machinery or engage in strenuous activity  or cook for 24 hours after your procedure.
  • Do not bathe until the day after your procedure.
  • Resume pre-injection medications.
  • Care of injection site: If you experience any soreness or pain, apply ice to this area today and warm moist heat tomorrow.  Notify Foothills Pain Management Clinic at 626-608-7320 if the injection site becomes reddened, swollen, hot or painful, or begins to bleed or drain.
